Introduction to Request Forms

Request Forms allow you to collect structured information from users through Requests. You can use them to gather updates, evaluations, reports, follow-ups, and other types of responses from the right audience.

This guide walks you through how to create and configure a Request Form in AcceleratorApp.

Where to find Request Forms

To create a Request Form:

  • Navigate to left sidebar menu by clicking on top left corner.
  • In the left sidebar menu, click Forms Management.
  • Next, click on the blue Create New Form button on the top right of the screen.

Step-by-Step

1. Creating a New Request Form

To create a new Request Form:

  • Once you've clicked on Create New Form.
  • Enter a clear Form Name.
  • Select the Request form type.
  • Select Language(s) (optional).
  • Then press Create to begin configuring it.

Use a form name that clearly describes its purpose so it is easy to identify later.

2. Adding Questions

After creating the form, add the questions you want recipients to answer.

You can add the fields needed to collect the right information. Use the Drag and Drop functionality to add different types of questions:

  • Text fields (Long and One Liner questions) – For open-ended feedback
  • Multiple choice – To categorize responses
  • Rating scales – To quantify satisfaction
  • Yes/No questions (Single Choice question) – For quick binary answers
  • Attachment questions – To allow users to upload supporting files
  • Number questions – For numerical values only
  • Date questions – To collect date-based responses
  • User Add Row questions – To allow users to answer the same question for multiple topics by adding additional rows

Other supported field types available in your form builder

When building the form:

  • Add each question you want users to complete.
  • Give each question a clear label.
  • Mark questions as required when a response must be completed before submission.
  • Arrange the questions in a logical order.

3. Configuring Form Fields

As you build the form, in the right side panel, you can configure each field based on the type of information you want to collect.

Depending on the field type, you may be able to configure settings such as:

  • Field label
  • Help text or description
  • Required or optional status
  • Options
  • Validations
  • Conditional rules

Use clear labels and instructions so recipients understand exactly what is being requested.

4. Using Conditional Logic

If supported in your field, you can use conditional logic to show or hide questions based on previous answers.

This is useful when:

  • A follow-up question should only appear if a user selects a certain answer
  • Different audiences may need to answer different questions
  • You want to keep the form shorter and more relevant

Before publishing the form, review any conditions carefully to make sure they behave as expected.

5. Reviewing and Saving the Form

Before using the Request Form in a Request:

  • Review all questions and settings
  • Check the order of the fields
  • Confirm required fields are correct
  • Make sure the wording is clear and consistent
  • Review all your conditional rules and validations
  • Save your changes
  • And then press Publish

A well-structured form helps improve completion rates and reduces confusion for recipients.

6. Using the Form in a Request

Once the Request Form is ready:

  • Go to Request
  • Create a new Request
  • Select your Request Form in the Form field
  • Choose the correct target audience
  • Save and continue with the Request setup

After that, the form can be sent to the selected recipients as part of the Request.

Tips and Best Practices

  • Keep forms as short as possible while still collecting the information you need.
  • Use clear and direct question labels.
  • Only mark fields as required when necessary.
  • Group related questions in a logical order.
  • Test the form before sending it to a large audience.
