How to Create a Feedback Form


Introduction

Feedback is essential for improving programs, services, and user experiences. AcceleratorApp's Form Builder allows you to create Feedback Request Forms, enabling you to collect valuable insights from applicants, startups, mentors, and other stakeholders.

 

Feature Location

To create a Feedback Request Form, navigate to the Side Bar Menu > Forms Management, click on Create New Form, name your form, and select Feedback as the form type. If needed, add additional languages from the dropdown menu.

Step-by-Step Actions

1. Adding Questions

Use the Drag and Drop functionality to add different types of questions:

  • Text fields (Long and One Liner questions) – For open-ended feedback
  • Multiple choice – To categorize responses
  • Rating scales – To quantify satisfaction
  • Yes/No questions (Single Choice question) – For quick binary answers
  • Attachment questions – To allow users to upload supporting files
  • Number questions – For numerical values only
  • Date questions – To collect date-based responses
  • User Add Row questions – To allow users to answer the same question for multiple topics by adding additional rows

 

2. Formatting Elements

To improve form organization and readability, add:

  • Section Headers – To divide the form into logical parts
  • Subheaders – To introduce specific sections
  • Paragraphs – To provide additional context or instructions

 

3. Setting Validations

Ensure you receive high-quality responses by setting validations:

  • Character limits for text responses
  • Required fields for essential questions
  • Email format validation for contact information
  • Numerical ranges for number questions
  • Date restrictions for date-based questions
  • Etc: it will depend on the type of field you are working on

 

4. Applying Conditional Rules

Use conditional rules to tailor the form based on responses:

  • If a user rates a service poorly, trigger an open-text field asking for improvement suggestions.
  • If a user selects "Mentor" as their role, show mentor-specific questions.

 

5. Converting Fields into Custom Fields

You can share the application form as a preview link that allows external users to visualize the form without needing an account. However, this link is for preview purposes only and does not allow submissions.
